How did the disagreements among the reviewers get resolved? Or if you were unable to reach consensus, what were the main sticking points?

The reviews were fairly consistent, so there weren't many disagreements to be resolved. Some of the main points largely agreed on by the reviewers and during the discussion are:

• Defining fairness as TCP-like behavior may not be the best decision.
• The authors did a good job of surveying a number of protocols in collecting their data.
• The authors did an adequate job of proving that misbehaving receivers cause problems, especially since they quantified their results, although there was a fair amount of opinion that the general conclusions reached were rather obvious.
